Skilled Nursing Facility Standardized Quality Scorecard

The Metro Alliance of Geriatric Providers is excited to announce the release of the Skilled Nursing Facility (SNF) Quality Scorecard. This scorecard will be used by Metro Alliance partner facilities in an effort to standardize the reporting of quality data.

This scorecard was developed as Metro Alliance members are requesting and need SNF quality data from facility partners. SNFs are reporting the requests from health systems have varying metrics, definitions, and reporting periods. Lack of standardized measures and data has resulted in SNFs assuming a significant administrative burden. Additionally, health systems report difficulties in collecting responses to data requests and question the validity of information.

In January 2016, the Metro Alliance recommended a small group of providers create a uniform set of commonly defined quality measures, benchmarks, reporting period and data collection template. The SNF quality scorecard is the result of this recommendation. SNFs will record the data measures that were agreed upon by the small group and approved by the Metro Alliance. Health systems can request that data, and the SNF will have a standard scorecard that they will present. This will ease the administrative burden for SNFs and give health systems the opportunity to compare quality data for many SNFs at once.

The scorecard will be an important tool as we continue to work on improving care across the continuum, from acute health facilities to post-acute facilities. These partnerships are vital to successful patient care and help improve the quality of the care that is delivered to our patients.
