Development of Common Quality Metrics
|
For SNF partners – discussed below metrics that many of
our groups are currently asking facilities to collect (Falls with major injury, LOS,
readmission rate, facility acquired infection rate, ED visit rate, community
discharge rate, med error rates, new or worsening pressure ulcers (CMS
definition), improvement in ADLs, deficiencies (f-tags))
-Discussed how the new CMS impact Act (2017) will change
the above
-Further discussion about other metrics than above to
different quality metrics not collected by CASPER (turnaround time for
orders, turnaround time for pharm for admission, ABX turnaround time, etc)

Facility request for provider order entry into
Facility EMRs
|
Many of us are being
asked to document in facility EMRs.
Noted opportunity:
potential time saver in some areas once fully interoperability
Noted Burden: time
burden on clinician (doing it, learning each new system), high risk for error
due to high variability facility use of EMRs across the metro, Risk of error
with call/coverage, less facility
nurse double checks.

Unified response remains ongoing discussion: At this time the Metro Alliance of
Geriatric Primary Care provider organization groups acknowledges support for
adopting new technology and processes that improve patient care and safety,
and also improves care effectiveness and efficiencies for all. At this time
there are significant concerns that the technology and processes currently
considered may pose risks to patient care and safety and are shifting costs
and inefficiencies to medical providers. Although welcoming new technologies
in improved communication such as secure texting, the current status of
provider order entry needs further dialogue and workflow development that
require provider input to assure that it is beneficial to all involved, while
improving care and safety. At this time the providers from Metro Alliance
organizations do not recommend participating in training or implementation of
provider order entry at nursing facilities until such mutual benefit is
established. We look forward to continued advancement in this area.

will proceed with next steps on tool kit.

New Metro Alliance project: High risk
Medication Eductional Tool Kit
|
Vanessa Kasper is a new U of M DNP student working with us
in this area
Discussion about needs, goals, outcomes.
|
Encouraged project to focus more on managing high risk
medications vs identifying/”not use” etc.
Vanessa will reach out to groups who noted interest to
interview – come back next meeting with next steps.
