Position paper was sent to group prior to meeting for
final review; no further enhancements at meeting.
Detailed review of 4 case studies with changes,
recommendations provided by group

any final thoughts or changes to Vanessa asap; the education case studies
will go live end of September.
Share
the education case studies with colleagues, new graduate NPs, etc
Vanessa will reach out to each group to give a leader guide and
offer support with sharing the modules.

After Hours Coumadin/INR Management

Lindsey Pordoehl and Laurie Wenker
DNP student taking on this project. After Hours Protocol
previously developed by this group needs updating with facilities now all
using electronic medical administration records. Little discussion about how
to get After Hours protocol into computer orders to minimize after hour calls
and promote continuity of care
|
More discussion to come. Share any tips/guidelines, etc
that are working for your practice with Laurie Wenker.

CPOE (Computerized Provider Order Entry) Work group
|
Dr. Downing, Holly
Gustav, Veronica Polinske, Dr. Mielke. Workgroup met, discussed. Discussion
around issues: each facility has own process, time commitment by provider to
do double entry of orders into facility chart and provider chart, not all
facilities use electronic chart the same (ie: Point Click Care is used
differently at different facilities). Complex issue across many facilities,
provider groups, especially with no facility standardization
|
Planning A3 strategy session to develop an organized
plan if this could be moved forward.
Stratis may be able to assist with group work

Patient Satisfaction Survey

Update: Allina,
Fairview, HealthEast, and Health Partners developed/agreed upon 3 TCU patient
satisfaction survey questions to be included in each organization’s patient
satisfaction survey starting 1/2017. This will provide benchmark for each
organization to compare themselves to other similar organizations.

Development of Common Quality Metrics/
Definitions for SNF partners
|
Julia Christ
Discussion around developing
a common scorecard for SNF quality metrics. Many organizations asking SNFs
for a variety of quality metrics; would like to develop a standard “ask” to
minimize the workload on the SNFs. Quality metrics now being used by a variety
of organizations, contracts, ACO, Medicare/Medicaid for payment as well as
measuring quality.

Starting a C.diff
research project; have 46 participating facilities
across the state;
part of a national study.
