1.      Welcome/ Introductions
·         21 total attendees ; 19 in person and 2 phone
·         11  health care provider organization practices being represented: Allina, Fairview, Genevive, Health East, Health Partners/Park Nicolette, HCMC,  Integrated Care by Medica, LTC Professionals, Optage, Twin Cities Physicians
·         2 health organizations represented: Care Choice,  Stratis

3.      Current Metro Alliance Projects
a.      Computer Provider Order Entry (CPOE)
Emily Downing
·         Updated the group on the progress of CPOE work group. Order form has been implemented at 4 sites: Langton Place, Homestead at Anoka, Johanna Shores and Folkstone.  Timeline for implementation at other Presbyterian Homes (Bloomington) has been extended into 2018 due to competing priorities.
·         Goals of the project include:
o   Improving first pass completion for orders – driving to a more consistent order writing process (including diagnoses and other order details)
o   Decreasing variability in order writing process for providers in multiple facilities
·         Tool kit is available and ready for additional site dissemination in or outside of Presbyterian Homes. The order form and provider order work flow process in posted online.
·         Meeting with Stratis Health this week- Jodi Winters to consider possible Webinar for SNF sites to introduce the process more globally
·         Demo from PCC possible.  Dr. Downing to connect with PCC at AMDA

b.      Antibiotic stewardship project
Cassandra Parker moving to Jessica Phelps
·         Partnership between MDH, Stratis,  and Metro Alliance with the
·         Jessica will move the project to stage II which will include a provider module
·         Jessica and Ali will place a Antibiotic Stewardship page on the Metro Alliance page with a link to the Topics article
c.       SHO
Alison Romstad, Cathy Lauring, Cheri Montgomery, Janine Nelson
·         We added all three sets to web page
·         We updated our introduction paragraphs to highlight giving the Metro Alliance credit

·         Highlight when the yearly revisions will be made (January)
·         Each group/provider continues to work with your facilities to verify you are working with the most up-dated copy and giving the group credit.
Professional Groups
Group discussion about Metro Alliance merging with MAGIC
·         Bring back to MAGIC to have this as a break out discussion group at the fall conference
·         Continued current  work and thought process about the Metro Alliance being an arm of MAGIC
Clinical Issues
a.  Uniform approach to influenza treatment and prophylaxis

Group discussion of the variation of group protocols/treatments (lack thereof) and barriers both in SNFs and ALs

·         Work to find a DNP (Ali to Dianne W (U of M) and Mary B (St. Cates), Pharm D (Ali) and Gero Fellow (Greg) student to start a new Metro Alliance project
b.      Narcotics After Hours

Group discussion found two subgroups:
1.      Continued work with SNF/Pharms for afterhours refills
2.      Larger picture of Opioid Epidemic and how the SNF/post-acute word play a part
·         Dr. Mittal and Amy Tong to continue work with Care choice and consultant pharm D
·         Work to find a DNP/Pharm D and Gero Fellow (Greg) to start a new Metro Alliance project

c.       Anti-psychotic Rules
·         Current groups state all doing well
·         Kristi Wergen, Stratis: Noted Q3 2017 data showing a decrease to 13.5%  and has a list of 35 MN nursing facilities that haven’t shown a decrease or changes in their anti-psychotropic meds.

·         Providers present noted they can possible help those facilities by reaching out to the current medical director to help change culture.
d.      Other clinical areas brought up
·         The role of the consulting Pharm D vs the regulatory Pharm D in SNFs. Billable vs not
·         Care Choice 72 hr pharm D review discussion (barriers, needs)
Dr. Mittle will reach out to Consulting pharm D and gather more information about role/billing.
