We noticed a variance in influenza monitoring, treatment and chemoprophylaxis throughout facilities and health care systems. Patients would be better treated if there was a standardized process. A standardized process would shorten the time of drug to patient during confirmed cases and outbreaks in facilities.
We worked with a DNP student from the University of Minnesota. Current policies of facilities and health care systems were reviewed and reconciled to start the project. Literature was reviewed and drafts were vetted by the providers of Metro Alliance multiple times. We are pleased to submit the current documents and look forward to any future revisions to improve on the protocol.
We are using GFR for calculations in an effort to facilitate rapid and efficient administration of oseltamivir (Tamiflu ®) during an influenza outbreak. We had a robust discussion about the use of creatinine clearance vs. GFR to dose our elderly residents with renal insufficiency. After careful consideration, we are recommending the use of the GFR. Rationale for this decision is based on:
* GFR allows for earlier initiation of treatment as it is widely available in records, and early initiation leads to improved outcomes* the national kidney foundation supports the use of GFR* all means of estimating renal function in elderly is relative inaccurate compared to younger populations1* GFR closely approximates the creatinine clearance in most patients* providers are welcome to adjust the dose to creatinine clearence if appropriate
We are recommending using GFR as the community standard and believe the benefits of using GFR versus creatinine clearance outweigh the risks in this particular population. The participants in Metro Alliance and MAGIC are in agreement with this decision, and expect that this will not negatively affect our care centers compliance with state or federal regulations.
